MBBS in Italy vs India is a choice between two good routes, not a contest. This page compares the English-taught Medicine and Surgery degree at Italian public universities with MBBS in India on ten points: fees, places and competition, entrance exam, length, language, clinical training, licence path, living cost, scholarships and careers.
Indian figures come only from National Medical Commission (NMC) and National Testing Agency (NTA) documents. Where we could not cite an Indian figure, we describe the point without a number and say so.

The IMAT 2026 was held on Tuesday 29 September 2026. Next come anonymous scores on Thursday 8 October, the paper view on Monday 19 October and the national ranking on Monday 26 October.
In India, NEET (UG) 2026 results came out on 16 July 2026. The NMC seat matrix for 2026/27, as on 18 September 2026, is still being updated, so the seat total may change before counselling ends.

MBBS in Italy vs India: which is better for an Indian student?
It depends on your NEET rank and where you want to practise: MBBS in India is shorter and leads straight to practice in India. MBBS in Italy gives a six-year public degree in English at low fees, but a graduate returning to India faces the NMC's foreign graduate steps. The table compares both on ten points for 2026/27.

How do MBBS fees in Italy compare with India?
In Italy, fees are low at public universities and set by each one. For a family whose income is in India, the first-year fee on the cheapest route runs from €146 (₹16,000) at Pavia to €3,198 (₹3.5 lakh) at Padua in 2026/27. We cite no Indian fee figure, because fees vary by college and state.
The table shows each Italian university with places for non-EU applicants living abroad: its places, the fee for a family in India, its DSU body and the monthly budget in its city. Florence has no non-EU places this year, and Tor Vergata's course in Tirana is not in our fee data.

Is it easier to get an MBBS seat in Italy or in India?
Neither is easy, and the numbers are not directly comparable. India offers 1,40,664 MBBS seats for 2026/27 against close to 20 lakh NEET (UG) 2026 candidates. Italy offers 525 places to non-EU applicants living abroad, and in 2025 Catania admitted 60 of the 402 non-EU candidates on its list.
Our calculation gives about 7 Indian seats for every 100 NEET candidates, and about 15 admissions for every 100 ranked candidates at Catania in 2025. At Messina the first allocation took 56 of 341, at a last admitted score of 58.20 out of 90. Bari admitted 11 of 43 at 50.90 on its corrected list.
The two figures describe different pools. NEET candidates compete nationally for many kinds of seat, while each IMAT candidate living abroad competes only for one university's places, chosen at pre-enrolment. Our IMAT cut-offs by university and good IMAT score guide show where the lines fell.
Do Indian students need NEET for MBBS in Italy, and how does the IMAT differ?
Yes, NEET matters for Italy too. Italian universities do not ask for it, but under the NMC's Screening Test Regulations an Indian citizen or OCI holder starting a medical degree abroad on or after May 2018 needs to have qualified NEET-UG to practise in India later. The NMC says the result stays valid for three years.
So an Indian student aiming for Italy plans for both tests: NEET (UG) in May or June and the IMAT in September. In 2026, NEET had a first sitting on 3 May and a re-examination on 21 June. How long is MBBS in Italy compared with MBBS in India?
MBBS in Italy takes six years and 360 ECTS credits, with a three-month internship inside the course. MBBS in India takes four and a half years of study, then a one-year compulsory rotating internship: about five and a half years in all. A graduate returning to India then adds 12 months of supervised internship there.

How does clinical training in Italy compare with India?
Both degrees build clinical training into the course, in different shapes. The Italian degree gives at least 60 credits to professional training and includes a 15-credit practical internship of three months. The Indian degree runs four phases of study and then a 12-month rotating internship after the final exams.
That difference matters for the NMC. Its FMGL 2021 rules ask for a 12-month internship in the same foreign institution, in addition to the course, and the Italian degree has no separate 12-month internship after graduating. We found no NMC document on how it treats this for Italy.

Can you practise in India after MBBS in Italy?
Yes, if every NMC condition is met, and the NMC decides that. The FMGL 2021 rules ask for a course of at least 54 months taught in English, a 12-month internship abroad, a licence to practise in Italy on the same terms as its citizens, the licensing exam and 12 months of supervised internship in India.

How much does living in Italy cost compared with India?
In Italy, our city guides put a student's month at €500 (₹55,000) in Messina up to €1,200 (₹1.3 lakh) in Rome and Bologna. The study visa asks proof of at least €10,179.85 (₹11.2 lakh) a year. We cite no Indian living-cost figure, because it depends on the city and on college hostels.

The main one is the regional DSU scholarship, awarded on family income rather than marks. For 2026/27, the national rules cap the family ISEE at €28,339.88 (₹31.1 lakh) and set a grant of at least €7,171.11 (₹7.9 lakh) a year for a student living away from home. Winners are usually exempt from the university contribution.
At the medical universities, family ISEE limits run from €22,500 (₹24.7 lakh) at ERSU Messina to €28,339.88 (₹31.1 lakh), and the top grant reaches €8,248 (₹9.1 lakh) at Milan and Milano-Bicocca. When is MBBS in India the better choice?
India is the better choice when your NEET rank wins a seat you can afford and you plan to practise in India. The degree is shorter, graduation leads straight to practice in India, and there is no foreign graduate exam or extra internship. Teaching can also run bilingually in 11 regional languages alongside English.
- A government or affordable seat: 64,121 of India's 2026/27 seats are in 447 government colleges, per the NMC seat matrix.
- A career planned in India: an Indian graduate skips the screening test, the NMC's foreign graduate conditions and the 12 months of supervised internship an Italian graduate would add.
- No new language: an Italian degree needs Italian for the clinical years, with patients.
- Family close by: six years in Europe is a long time away, and the visa file needs proof of at least €10,179.85 (₹11.2 lakh) a year.

What are the limits of this comparison?
The main limits are missing Indian figures and rules that can change. We cite no Indian fee, living cost or scholarship figure, because we could not open an official page giving one. The NMC seat matrix is still being updated, and the National Exit Test, once in force, would replace the screening test.
Reality check
- Italian places are few. 525 places for non-EU applicants living abroad in 2026/27, and each candidate competes for one university only.
- The NMC internship question is open. FMGL 2021 asks for 12 months of internship abroad in addition to the course; Italy's degree has a three-month internship inside it.
- Italian is needed. Teaching is in English, but patients speak Italian in the clinical years.
- A scholarship is not certain. Eligible students can still miss out when funds run short, and the application comes before the IMAT result.
- Some fees rest on older rules. Vanvitelli and Sapienza cells use 2025/26 amounts, and Bari's country-group fee could not be read.

For the lessons, no: the 17 courses in our guides teach and examine in English, as the NMC's FMGL 2021 rules require of a foreign degree. For the clinical years, yes, because patients speak Italian, so Italian is needed before the clinical years begin.
Is an MBBS from Italy valid in India?
It can be, if every NMC condition is met, and the NMC decides that. The six-year English-taught degree meets the length and language rules. The condition to weigh most is the 12-month internship abroad, and we found no NMC document on how it treats Italy's three-month internship.
What IMAT score do Indian students need for MBBS in Italy?
It depends on the university. In 2025 the last non-EU candidate admitted scored 61.60 at Catania, 58.20 at Messina in the first allocation and 50.90 at Bari, out of 90. Each non-EU candidate living abroad competes only for the university chosen at pre-enrolment.
