School-leaving diploma
For the 2026/27 intake you needed a secondary school diploma, already held or to be obtained in 2026. If your diploma comes from a foreign school system, it must meet Sapienza's list of required documents for foreign qualifications. [1]
The call does not set a minimum school grade. Places go by your score in the admission test. [1]
What this means for your school results
As a guide, based on the rule above:
- Usually a fit: Class 12 or Higher Secondary in the science stream with physics, chemistry and biology, or A levels in biology and chemistry. 51 of the 60 test questions are on biology, chemistry, physics and maths.
- Check carefully: a science stream without biology, such as PCM, or with only one of biology and chemistry. Your diploma can still meet the rule, but 23 questions are on biology and 15 on chemistry.
- Often short: commerce or arts stream results. The diploma rule may be met, but the test needs school-level biology, chemistry, physics and maths, so expect long preparation.
Students admitted with low biology or chemistry scores must clear extra learning requirements in their first year (see below). [1]

The whole test is in English, including the reading questions. The call does not list an English certificate among the entry requirements. [1]
A recognised English certificate does count as a tie-breaker in the ranking if you hold it by 9 September 2026 and declare it when you register. Listed certificates include: [1]
- IELTS: band 5.5 to 6.5 (B2), 7.0 to 8.0 (C1) or 8.5 to 9.0 (C2)
- TOEFL: 87 to 109 (B2) or 110 to 120 (C1)
- Cambridge First, Advanced or Proficiency, PTE General Levels 3 to 5 and Trinity ISE II to IV
If you attended a school where English was the language of instruction for at least one academic year, you are automatically treated as holding a certificate. [1]
Admission test and places
The test is the same across Italy and is prepared by the Ministry of University and Research. It has 60 multiple-choice questions with five options, in 100 minutes: [1]

- 5 on logical reasoning and problems
- 23 on biology
- 15 on chemistry
- 13 on physics and mathematics
Each correct answer scores 1.5 points, a blank scores 0 and a wrong answer loses 0.4. [1]
For 2026/27 there were 65 places: 52 for EU and equivalent applicants and 13 for non-EU applicants living abroad who need a student visa. [1]
Ranking and minimum scores
- Non-EU applicants living abroad: a score above zero makes you eligible. Sapienza draws up your ranking and publishes it on the degree page. [1]
- EU and equivalent applicants: a score of at least 20 makes you eligible for the single national ranking. [1]
- Extra learning requirements (OFA): if your total is below 30 and you scored under 8.6 in biology or 5.6 in chemistry, you must pass that subject's first-year exam or a remedial test within your first year. Otherwise you repeat the first year. [1]
Ties are broken by subject scores in the order biology, chemistry, physics and maths, logic, then reading, and then by a declared English certificate. [1]
2026/27 deadlines
These were the dates in the 2026/27 call: [1]
- 30 June 2026: Universitaly pre-enrolment for non-EU visa applicants.
- 26 August to 9 September 2026 (3 p.m. Italian time): test registration on Universitaly, choice of test location and fee payment. Registration with Sapienza's Infostud system, from 7 August to 9 September 2026, was also needed to sit the test at Sapienza.
- 29 September 2026: admission test, in Italy and at centres abroad.
- 8 October 2026: anonymous scores published; 26 October 2026: national merit ranking; 3 November 2026: next ranking update.
- After each ranking you have four days to enrol if assigned a place, and five days to confirm your interest in staying in the ranking. Both count the publication day but not Saturdays or public holidays.
For 2026/27 the test was also held abroad, including in India, Pakistan, the UAE (Dubai), Saudi Arabia, the UK and the USA. Non-EU visa applicants who sit the test in Italy must choose the location of the university where they pre-enrolled. [1]

Enrolment and health checks
You enrol by paying the first fee instalment and the regional tax. With a foreign school qualification, you also send the documents on Sapienza's list to its international student office by the enrolment deadline. [1]
Within 60 days of paying the first instalment you need a Mantoux tuberculin test and vaccination against hepatitis B, both at the local health authority, with a medical certification form signed by a doctor. [1]

About the programme
Medicine and Surgery "F" (Medicina e Chirurgia "F") is a six-year single-cycle master's degree in class LM-41, worth 360 ECTS and taught in English in Rome. It is run by the Department of Anatomical, Histological, Forensic and Orthopaedic Sciences and shared by Sapienza's Faculties of Pharmacy and Medicine, Medicine and Dentistry, and Medicine and Psychology. Access is by a national admission test: for 2026/27 there were 52 places for EU and equivalent applicants and 13 for non-EU applicants living abroad. [1]
Programme F is open to applicants from the European Union and from many non-EU countries, and aims to build an educational community that values multiculturalism and cross-cultural communication. It suits students who want to train as doctors in English and are ready for intensive study, attendance and ward work.
What you will study
The early years cover the basic sciences, followed by pathology, laboratory medicine and clinical methods, and then applied pathology by organ system and the clinical specialities. A Medical Humanities curriculum runs alongside, and a unit in Italian language skills starts in the first year. Units in the study plan include:

How the programme is taught
Sixty ECTS are devoted to practical training, split into clinical internships and qualifying internships. From the first years students use laboratory instruments and attend the wards of the affiliated teaching hospitals, Policlinico Umberto I and Sant'Andrea. From the second year they can use the simulation skill lab through the BLS course, and in the third year they practise physical examination on peers and patients.
To help students learn Italian, third-year students work in Italian with a professional actor playing a simulated patient. From the fourth year they train on simulators and spend two consecutive weeks in emergency-related wards with clinical tutors, at a ratio of one tutor to one or at most two students.

Graduates have the scientific and practical training to practise medicine in line with Directive 75/363/EEC. The university lists these paths:

Fees and scholarships
Sapienza sets tuition by family income (ISEE). Students whose income comes from abroad and who cannot provide an ISEE pay a fixed yearly amount based on their country, set in each year's fee regulations. Medicine and Surgery "F" does not use Sapienza's MoveIn pre-selection. Entry is by a national admission test in English, set by Italy's Ministry of University and Research, for a limited number of places. For 2026/27, non-EU visa applicants completed Universitaly pre-enrolment by 30 June 2026, registered for the test on Universitaly between 26 August and 9 September 2026, and sat it on 29 September 2026 in Rome, another Italian location or a centre abroad. Non-EU applicants living abroad have their own ranking, drawn up by Sapienza, and need a test score above zero to be eligible. Successful candidates enrol by paying the first fee instalment and apply for a type D study visa.
